About the Program
We consult with a variety of other programs and departments at DHMC in Lebanon, NH and regionally. We provide a full array of community-based services not only in clinical settings, but in collaboration with the schools and towns where children live.
The family/guardian's written permission is required before we can communicate with the child's primary care provider and school.
Inpatient and outpatient services
- Our staff works closely with each child's primary care physician to assure the best communication and continuity of care.
- A comprehensive summary report — including an updated diagnosis and problem/issues list — is prepared following each visit, and sent to each child's family/guardian and primary care physician. The report will be available to the referring provider, as well as to others authorized by the family.
