What to Expect Going Home
The Healthcare Team, including the family, will carefully consider the anticipated needs of the patient, updating those plans during the course of the hospitalization.
On the day of discharge, the doctor will complete the patient’s discharge paperwork which includes
- Plan for follow up with the primary care physician and/or specialty service
- Prescriptions for medications
- Any special care instructions for the patient and/or a Home Care Agency
Your nurse will review these instructions with the patient and family and provide a written copy.
Our goal is for patients to be discharged in the morning, and we will do our best to make that possible for all. Unfortunately, sometimes there are unexpected delays, and for that we apologize. We want you to know that CHaD is actively working on improving the coordination and communication between patients, families, and the healthcare team on the day of discharge.

After you go home, you may receive a telephone call or a survey in the mail, asking you about the care you received while in the hospital. Please help us by answering the questions. Information from these surveys helps us to continually improve care.
