Immunizations
The Consent to Treatment Form (PDF) must be filled out and handed to the receptionist on the day of the appointment. Please read the appropriate Vaccine Information Statement (VIS) as this has very important information about the immunizations your child may be receiving. The VIS sheets may be printed off for your own records but do not need to accompany your child to his/her visit.
